This outbreak, caused by the rare Bundibugyo virus, has been particularly challenging due to a delayed response and a complex, conflict-ridden environment. While the number of cases is believed to be higher, the contact tracing coverage rate remains low at 64%, highlighting the ongoing struggle to contain the spread.
What makes this outbreak particularly concerning is the skepticism and disregard for health protocols among some locals. Survivors of Congo's 2018 Ebola outbreak have warned that a repeat of past mistakes could lead to a high number of preventable deaths. This raises a deeper question: How can we effectively communicate the importance of health measures and build trust with communities in the face of such challenges?
One thing that immediately stands out is the impact of conflict on the response efforts. Eastern Congo has seen attacks by dozens of separate rebel and militant groups, some with links to foreign countries or the extremist Islamic State group. This conflict is 'constraining access for the response, disrupting surveillance and response activities, and increasing the risk of undetected transmission,' according to the World Health Organization. The displacement of nearly a million people due to conflict further complicates contact tracing, as people flee attacks or move frequently in the vast province with dense forests, poor roads, and remote villages.
The rapid increase in the number of cases is also due to the scale-up of diagnostic capacities, enabling testing of previously collected samples. However, this also means that the actual number of cases may be higher than reported. The outbreak has been concentrated in the eastern province of Ituri, which accounts for more than 90% of the cases, with cases also recorded in North Kivu and South Kivu provinces, and spreading across the border to Uganda.
In my opinion, the key to containing this outbreak lies in building trust and understanding with the local communities. The skepticism and disregard for health protocols among some locals highlight the need for a more nuanced approach. We must recognize the cultural and social factors that influence people's behavior and work closely with local leaders and communities to develop effective strategies. This includes addressing the underlying causes of conflict and displacement, as well as providing education and resources to support contact tracing and other response efforts.
